Sports Information

To Participate in Sports, athletes must have:

  • Current physical on file in the health office. Physical must be dated within 1 year of the month that the Sport begins.
  • Medical Update and Recertification. For every sports season an athlete participate in a medical update needs to be submitted through familyid.com.  The website needs to be completed and  electronically signed by a parent within 30 days of the start of the season  The athlete should check in with the nurse to make sure physical is up to date and process has been completed.
  • Medication form signed by parent and physician stating that athlete can self carry and self-administer medication. Required for all athletes who will carry/administer their own medication during sports participation or who require emergency medications.

Athletic Participation in East Rochester

Per NYS guidelines, the following forms need to be submitted for review by the School Nurse for you to be medically cleared to participate in athletics.

  • All athletes need to have an updated physical on file in the Health Office. The physical must be dated within 12 months of the start date of the sport.
  • Any athlete who will carry/administer his/her own medication during sports participation is required to have a medication form on file in the Health Office. The form, which must be signed by both a physician and a parent, needs to state the student can self-carry and self-administer medication. The form is valid for one school year and must be renewed each year.
  • The Medical Update and Recertification form needs to be completed and signed by parent/guardian for every sports season a student athlete participates in. The form needs to be completed and handed in to the School Nurse within 30 days of the start of the season.

A physician’s note is required to clear an athlete for participation in sports for any injury or illness that occurred since the last physical and that led to the athlete missing more than five days of practice.

There is a new process for any seventh- and eighth-graders who wish to compete at the JV or Varsity level. The AAP or Accelerated Athlete Process is new this year; please contact the Athletic Office with any questions.
